ABSTRACT
Introduction: Pregnant women immunological response to influenza infection is considerably diferente than the general population. Effects of vaccination in pregnant women as well as their foetus are not fully understood. Objectives: The aim of this study is to perform a systematic review updating and expanding the best available evidence on benefits (safety and immunogenicity) of seasonal fragmented inactivated influenza vaccination, during any time of gestation for pregnant women and newborn. Methods: A systematic review was conducted through searching in Web of Science (WoS), Medline (via PubMed), Embase, LILACS/BVS and CINAHL from the onset of database until July 2018. Mendeley reference manager was used to remove duplicates. Two independent reviewers did the initial selection of papers by reading titles and abstracts and then by reading publications in full. Disagreements were resolved by arbitrage of a third reviewer. At this stage the site Rayyan QCRI was used. Data extraction will be recorded in a designed clinical spreadsheet and the most relevant results will be summarized in a narrative synthesis. The methodological validity and quality will be accessed by standardized critical appraisal tools from the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) guidelines. Results and Discussion: The protocol was registered in the International Prospective Register of Systematic Reviews - PROSPERO (https://www.crd.york.ac.uk/PROSPERO) # CRD42018104038. The search strategies were based on the defined acronym PICOS: (Participant) pregnant woman; (Intervention) seasonal trivalent fragmented innactivated influenza vaccine; (Comparator) according to studies found in the review; (Outcomes) local and systemic adverse event/reaction, significant immunogenic improvement and maternally- acquired immunity to influenza; (Study design) all types without restrictions. The search in the databases found 11460 studies and after the screening were included 118 studies for qualitative synthesis. The extraction of data was based on the 5 experimental clinical trials. The remaining observational studies are under analysis and will be included for future publication. The clinical trials evaluated here demonstrate that influenza vaccination in pregnant women protects mothers and newborns and lowers rates of low birth weight.
Introdução: A resposta imunológica das gestantes à infecção por influenza é consideravelmente diferente da população em geral. Os efeitos da vacinação em mulheres grávidas, assim como no feto, não são totalmente compreendidos. Objetivos: Realizar uma revisão sistemática atualizando e expandindo as melhores evidências disponíveis sobre os benefícios (segurança e imunogenicidade) da vacina TIV, durante qualquer período de gestação para gestantes e recém-nascidos. Métodos: As buscas foram realizadas nas bases, Web of Science, Medline (via PubMed), Embase, LILACS / BVS, CINAHL, Clinical Trials, Catálogo de Teses e Dissertações da CAPES e Biblioteca Digital de Teses e Dissertações da USP. A seleção dos estudos foi realizada por dois revisores independentes e um terceiro para resolver as discordâncias. Utilizou-se nesta etapa o site e aplicativo móvel para revisões sistemáticas - Rayyan QCRI. A extração de dados foi realizada por uma ficha clínica adaptada e os resultados mais relevantes foram resumidos em uma síntese narrativa. As diretrizes metodológicas de Avaliação, Desenvolvimento e Avaliação da Classificação de Recomendações (GRADE) será a ferramenta de avaliação crítica dos estudos. Resultados: O protocolo foi registrado no Registro Prospectivo Internacional de Revisões Sistemáticas - PROSPERO (https://www.crd.york.ac.uk/PROSPERO) # CRD42018104038. A busca nas bases de dados encontrou 11460 estudos e após as triagens foram incluídos 118 estudos para síntese qualitativa, que desses foi realizada a extração de dados dos 5 ensaios clínicos experimentais e os demais estudos observacionais estão em análise para inclusão e futura publicação. Os ensaios clínicos aqui avaliados demonstram que a vacinação de influenza em gestantes protege mães e recém nascidos e diminui as taxas de baixo peso ao nascer.
